Obtain a U of M Medical Record Number (MRN)
If you have not received care at the U of M before, or do not have a MRN, you can call Michigan Medicine Registration at (734) 936-4990 to obtain one. They are available M-F 8am-9pm (ET) and Sat 8am-12pm (ET).
Be ready to provide them with your contact info and insurance info. We will need your MRN to schedule your appointments.
Double-check Insurance Coverage
Please call your health insurance provider to see if our physician Dr. Scott-Craig at Michigan Medicine is considered in-network.Also, ask if they cover the following procedure codes and diagnosis codes:
- 99205 for new patient visit
- 99214 & 99215 for return visit Diagnosis (ICD-10) codes commonly used (not all will necessarily apply to you):
- Type 2 diabetes w/o complications E11.9
- Prediabetes R73.09
- Hypertension I10
- Hyperlipidemia E78.5
- Obstructive sleep apnea (OSA) G47.33
- Morbid Obesity E66.01
- Binge Eating F50.81
- Non-alcoholic fatty liver disease (NAFLD) K7.60
- Non-alcoholic steatohepatitis (NASH) K75.81
- Polycystic ovarian syndrome (PCOS) E28.2
In order to check for coverage to see the dietitians at Michigan Medicine, please ask your insurance these 3 questions:
1) Do you cover the diagnosis code for obesity (E66.09) with BMI code Z68.31, or Dietary counseling and surveillance (Z71.3)?
2) Do you cover the procedure code Intensive Behavioral Theory (G0447) OR Medical Nutrition Therapy (97802/97803)?
3) How many dietitian visits per year are covered?
Obtain a referral from your PCP (only if you have an HMO plan!)
If you have an HMO plan, you will need a referral from your Primary Care Physician (PCP) for insurance coverage. P
Please provide the attached form (Outpatient Consult Request) to your PCP and have their office fax the referral to U of M (fax # is written on the form).
